Jr.K-4 Transportation Notification Form

It is important that we know how your son gets to and from school each day so that we can be sure he gets home safely. Please fill out the transportation form below. When you complete the form please include specific information including school district and bus number (for those boys riding a public school bus) and the person who delivers your son and/or picks him up (for those boys going by car). In addition, on the rare occasion that your son misses his bus or his ride is delayed and we cannot reach you, please list the name and number of someone we can contact. Your cell phone number is also useful.
