HOW TO COMPLETE YOUR APPLICATION FILE:

Evaluation forms are an integral part of the admission process. During the application process, you can easily add teacher contact information which will allow teachers to directly submit the necessary information.
3)School and Transcript Request Form
Please fill out the "Authorization for Release of Records" portion of the "School Report Form" and submit it to your son’s school. His school will complete the form and mail it directly to University School.

The Financial Aid Office at University School aligns with the mission of the school. We hope to provide deserving students an opportunity for an amazing educational experience that may otherwise not be obtainable.
