Media Release Form

Members of the University School staff may, from time to time, photograph and interview students for the School’s marketing and promotional materials. I hereby grant University School the irrevocable right in perpetuity to use, re-use, reproduce, publish, re-publish, post on a website, adapt, modify and edit the photographic image, name, video or likeness of my son(s), and information about my son(s), and any testimonials from my son(s) or other related materials, for the use and benefit of University School, including, without limitation, in University School’s publications, fundraising, marketing and promotional materials, in any media including online materials, without any compensation or right of inspection.

Members of the outside media occasionally contact the school to interview and photograph students for educational/athletic stories. I will allow the outside media to interview my son(s) and to use his/their name(s) and photograph(s) and information provided by my son(s) for stories related to University School.

I understand that clicking on "Submit" replaces my written signature and represents that the information provided is true, complete, and accurate. I also understand this information will be relied upon by University School and that it will only be used in a way that is consistent with the school's Privacy Policy at http://www.us.edu/privacypolicy.
